Overview
The Position:
Under the direction of Manager of Care Coordination, the Registered Nurse (RN) provides high-quality, compassionate, and patient-centered nursing care to participants in any setting including in the participants' home and/or a CEI clinic. This role ensures the delivery of safe, effective, and individualized care, following the participant’s care plan, physician’s orders, and regulatory standards. The RN communicates effectively with all members of the clinic team and other program staff to ensure that participants are receiving care that is appropriate and adheres to the participants’ plan of care.
The Home Health RN position is eligible for a $5,000 Signing Bonus.
The pay range for the Home Health RN position at Center for Elders Independence is $45.12 to $67.68 per hour. Hourly rate based on the market for the Home Health RN position, as well as experience, skills, abilities and work history.
Responsibilities- Assesses participants in clinic, or in participant’s home, for post hospital, SNF, and fall assessments for medication, compliance and disease management or as indicated.
- Monitors participants for any acute physical or behavioral changes and adjusts plan of care accordingly.
- Provides skilled nursing care to participants as needed in a timely manner.
- Provides health education, information and counseling to participants, family members and caregivers regarding participant’s disease management, medication adherence, care needs and home safety.
- Implements preventative and rehabilitative nursing procedures as appropriate for the participant’s care and safety and all other reasonably related jobs as assigned.
- Participates in the after-hours on-call rotation and must be available evenings and weekends as scheduled.
- Provides emergency response and triage guidance during on-call coverage, coordinating with clinical leadership as needed.
- Attends and contributes to IDT meetings in the absence of clinic-based RNs, offering clinical expertise to support care coordination, planning and problem solving.
- Conducts initial, semi-annual, and significant change of condition nursing assessments of participants in the clinic to evaluate their status and support development and maintenance of individualized care plans.
- Maintains accurate and timely documentation of care provided in the electronic health record (EHR) in accordance with PACE and regulatory standards.
- Possesses strong clinical skills and experience in complex wound care, infusions, venipuncture, vaccinations, complex respiratory care, genitourinary care and ostomies highly preferred.
- Knowledge and experience in diabetic foot care and advance wound care training and/or certifications preferred.
- Assures quality of care by adhering to therapeutic standards, making or recommending necessary treatment adjustments, following standards of care set by Board of Registered Nursing, Nurse Practice Act, and governing agency regulations.
- Actively participates in CEI’s quality assurance performance improvement program (QAPI) as assigned, measuring health outcomes against patient care goals and standards.
- Maintains the confidentiality of participant records, family information, company procedures, results, and other sensitive data.
- Maintains a safe working environment by following all CEI safety policies, procedures, and infection control standards.
- Participates in departmental meetings, trainings, and agency-required sessions.
- Performs other assigned duties, contributing positively to the care team and participant experience.
- Graduate from an accredited school of professional nursing
- Bachelor of Science in Nursing (BSN) preferred
- Minimum of one (1) year of experience in an acute care, skilled nursing facility, or ambulatory care setting, preferably working with a frail or elderly population
- Proficiency in using electronic health records and basic computer applications
- Must have a valid California driver’s license, motor vehicle insurance, and reliable transportation
- Current California Registered Nurse (RN) license in good standing
- Maintains eligibility for licensure by completing required continuing education units
