Job Description As a member of a health care team, the Social Worker provides appropriate intervention services to patients and/or clients and their families. Work typically involves assessment of patient and family needs, providing emotional support, and developing a plan of services, including advocacy, referrals, and practical assistance necessary to achieve desired objectives. Provides consultation, orientation, and training to health care staff on cultural and economic issues related to patient/client health needs.

Functions
  • Collaborates with health care teams to treat and plan for the social and medical needs of patients by consulting with providers and making referrals. Devises, develops and implements treatment plans as necessary.
  • Conducts assessment of patients to evaluate social, emotional and physical needs by applying social work theories and techniques; interviews and evaluates patients and family members. Identifies factors which may interfere with obtaining maximum benefits from treatment.
  • Develops and implements patient management plans, including coordination of referrals to specialty and social services, recording of progress notes, and providing continuity of care. Helps patients and families cope with environmental and psychological issues of illness by explaining options and treatments and making appropriate referrals.
  • Prepares and maintains records and makes written and oral reports documenting case activity concerning overall patient care in accordance with department and organizational policies and procedures.
  • Participates in monitoring and evaluating the quality and appropriateness of patient care.
  • Provides supportive care to assist patients and families in coping with presenting symptoms and problems such as illness, accepting medical recommendations, and loss of function. Provides education to patients and families around adaptation to the patient's diagnosis, illness, treatment, or life situation.
  • Performs other duties as assigned.
Qualifications
  • Education: Master’s degree in Social Work (MSW) from an accredited college or university.
  • Experience: Two (2) years of progressively responsible and directly related work experience.
  • Licensure/Certification: Valid license as a Licensed Clinical Social Worker (LCSW) issued by the California Board of Behavioral Sciences (preferred).
Knowledge, Skills, and Abilities
  • Apply theories, principles, and practices of medical social work and social services delivery systems in a hospital or medical center setting.
  • Gather data needed to assess needs of patients and families.
  • Manage a caseload, maintaining confidentiality, determining appropriate course of action in emergencies and stressful situations.
  • Provide written case narratives and reports.
  • Apply effective assessment, interviewing, and counseling techniques.
  • Know hospital and medical center resources to access and provide patient care needs.
  • Understand needs, problems, attitudes, and behaviors of hospital patients and their families.
Physical Requirements / Work Environment

General office environment. Outpatient clinical setting with ability to travel between work locations as needed. Reasonable accommodations will be made to enable individuals with disabilities to perform the essential functions of the job.

Compensation is based on the level and requirements of the role. Salary may also be determined by education, experience, knowledge, skills, location, and abilities, as well as internal equity and market data. Typically, new team members join at the minimum to mid salary range. Minimum to Midpoint Range (Hourly): $47.45 to $56.94.
