Position: Physical Therapist (SNF)

The Vineyards Healthcare Center, a high-performing 83-bed nursing home in Livermore, is seeking a dedicated Physical Therapist to join its team.

Responsibilities
  • Conducting patient evaluations.
  • Developing treatment plans.
  • Monitoring progress.
  • Educating patients and caregivers.
    • Qualifications
  • Physical Therapy degree.
  • State licensure.
  • Strong interest in geriatric care.
  • New graduates are welcome; experience in skilled nursing is a plus.
  • Strong communication and problem-solving skills.
  • EMR proficiency is desired.
