Physical Therapist for Home Health Visits

Base pay range: $90,000 - $170,000 per year (actual pay determined by experience and qualifications).

Apollo is recruiting for a Physical Therapist to join our Home Healthcare Team. In this role, you will provide quality home health visits to patients in the comfort of their homes, working with a dedicated, compassionate team committed to delivering the highest level of care.

• Provide physical therapy services to patients following a physician's plan of care, including:
  • Assess and evaluate therapeutic, rehabilitative, and functional status; participate in the development of the total plan of care; assess home environment.
  • Assess muscle strength, mobility, gait, and range of motion to determine rehabilitation potential.
  • Direct physical therapy treatment.
  • Instruct patients and caregivers on the use and care of therapeutic appliances.
  • Determine priority needs for physical therapy.
  • Report to physicians about patient reactions to treatment or changes in condition.
• Initiate physical therapy programs and instruct other personnel or caregivers in phases of care, participating in case conferences.
• Provide training to patients in prosthetic device use.
• Identify and refer patients and families to other home health services when needed.
• Prepare and submit clinical progress summaries based on goal attainment.
• Participate in discharge planning.
• Provide physical therapy consultation to families when indicated.
• Deliver in‑service education programs for nursing organization personnel.
• Participate in peer consultation processes.
• Supervise physical therapy assistants according to Apollo policy and state regulations.
• When physical therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two weeks.

• Degree from an accredited bachelor's or master's physical therapy program.
• Licensed to practice as a physical therapist in the state, if applicable.
• At least two years of relevant physical therapy experience.
• Excellent verbal and written communication and organizational skills.
• Current CPR certification.
• Willingness to travel to patients' homes.
• Discretion in handling sensitive and confidential information.

Prior experience in a home health setting is preferred but not required.

We offer competitive compensation: $90,000–$170,000 per year (actual pay varies by experience and geographic location). For in‑services, training, and orientations, the rate is $45 per hour.
