Baptist Health Home Health/Hospice RN Case Manager provides, manages and coordinates care and services for an assigned caseload of patients.
Qualifications- Graduation from an approved program of Nursing.
- Current Arkansas RN License.
- Current Arkansas Driver's License.
- Minimum 1 year of nursing experience.
- Home Health/Hospice experience (preferred).
This position will be authorized 80.00 hours bi-weekly.
