• Support the Care Management Department with non-clinical tasks related to discharge planning and utilization review
• Communicate effectively with post-acute facilities, patients, families, and UAMS team members to coordinate services
• Facilitate referrals to post-discharge providers, ensuring a smooth transition of care for patients
• Conduct follow-up calls to ensure receipt of necessary information from providers
• Arrange discharge appointments and ensure they are included in patients  discharge education

• High School diploma/GED with three (3) years of administrative/documentation experience, or an Associate s Degree with one (1) year of relevant experience
• At least two (2) years of customer service experience
• One (1) year of experience with medical records
• Strong communication skills and ability to work collaboratively with healthcare teams
• Flexibility to meet face-to-face with hospitalized patients as part of discharge planning

• Associate or advanced degree in a related field
• One (1) year or more of experience in healthcare settings
• Experience with registration and appointment scheduling using EPIC
• Knowledge of medical terminology
• Familiarity with durable medical equipment (DME) companies or post-acute care organizations
