• Developrelationshipsandcollaboratewiththeprimarycareteamstoconductsocialservicesassessmentstodeterminetheappropriateneedsforeachmember.
• Plan, coordinate, manage and implement support packages to help members deal with socioeconomic and medical barriers.
• Navigatemanagedcareplansforcommunityservicesandprograms.
• Casemanagementforsocialandbehavioralcaretoallowmemberstoself-managehealthandsocialservicesupport.
• Accurateandtimelydocumentationofpatientencountersandsessionsinallclinicalmanagementsystems
• Proactively identify methods to improve Arch Well Health ’s approach based on feedback and regularly conducted surveys.
• Support advance care transitions with members and their families
• Assist members with access to state-based prescription programs and other benefits.

• Experiencesupportingpatientswithneedforsocialservices.
• Experiencewithscreening,assessment,andplanningforcommonsocialservicesneeds
• Workingknowledgeofdifferentialdiagnosisofcommonmentalhealthconditions
• Stronginterpersonalcommunicationskillswithexceptionalactivelisteningabilities
• Highly empathetic,non-judgmental,andopen-minded
• Experienceinacollaborativeteamenvironment

• Fully licensed
  
  BSW orMSWinthedesired Stateofpractice
• Master’s degree in social work is preferred, Bachelor's is required
• 1+years' experienceinclinicalsocialwork
• Aproblem-solvingorientationandaflexibleandpositiveattitude
• Missiondrivenandmotivatedtojoinanorganizationthatwilltransformthewaywedeliveraccessible,clinicallyexcellentcaretoseniors.
• Fluencyin Spanishorotherlanguagesspokenbypeopleinthecommunitiesweserve(where necessary)
