We’re looking for a highly organized and experienced Clinical Coordinator (LPN) to join our dedicated team! This full-time, salaried role offers a consistent Monday–Friday schedule (8:00 AM – 5:00 PM), with an on-call rotation every other weekend and every other holiday.

What will I do in the role of a Clinical Coordinator (LPN)?

• Be responsible for the clinical oversight of Assisted Living and Personal Care units.
• Administer and document medications and treatments while monitoring residents’ health and responding to changes in condition.
• Maintain accurate electronic medical records and regulatory documentation, ensuring compliance with Department of Human Services requirements.
• Lead and support CNAs, providing guidance, monitoring performance, and promoting high standards of resident care.
• Coordinate clinical processes and audits, including e-MAR reviews and quality checks to ensure regulatory compliance.
• Collaborate with physicians, leadership, and staff to support resident care, new admissions, and ongoing clinical operations in Lititz.

What do I need in the role of Clinical Coordinator (LPN)?

• Must be a Licensed Practical Nurse licensed by the Pennsylvania Board of Nursing.
• Must be CPR Certified.
• One to two years of long-term care nursing experience preferred.
• One year of management training or experience preferred.
• Ability to use computers and software as applies to health care setting. Point Click Care experience preferred.
