Schedule: Sunday–Saturday 8:00 am–8:00 pm

As a Licensed Practical / Vocational Nurse, you will accompany a teenager to school with a tracheostomy or ventilator in place, providing in‑home care. You will perform nursing assessments, medication administration, wound care, trach care, and respiratory support while coordinating with the patient’s school team and healthcare providers.

• Provide skilled nursing care for a school‑bound teen with trach/vent care.
• Collaborate with school staff, parents, and healthcare professionals.
• Document care and update electronic chart using Aveanna tablet.
• Ensure compliance with Medicare/Medicaid regulations and health‑related requirements.
• Assist with transportation and travel logistics for school visits.

• Active, unencumbered LPN/LVN license in the state of practice.
• Compact license transferred within 90 days of employment, if applicable.
• Current CPR certification (hands‑on).
• TB skin test within last 12 months.
• Reliable transportation.
• Six months of hands‑on nursing experience preferred.
