Social Worker Opportunities Northumberland County Council
Contract Type: Permanent or Fixed Term (role-dependent)
Hours: 37 hours per week
Locations: Wansbeck General Hospital, Hexham, Morpeth, Berwick & Alnwick

Main duties of the jobSocial Worker Home Safe Hospital Discharge (Wansbeck General Hospital):
Complete urgent needs assessments, mental capacity assessments, and work with families and partner agencies to facilitate safe discharge from hospital.Social Worker West Care & Support (Hexham):
Join an integrated team supporting adults with complex physical and mental health needs. Manage own caseload with strong supervision and multiagency partnership working.Social Worker Prevention Team (Morpeth):
Undertake preventative, strengths-based assessments to support adults to maintain independence. Training opportunities include AMHP and Practice Educator pathways.Social Worker North Locality Care & Support (Berwick or Alnwick):
Support adults with physical disabilities and mental illness, working closely with primary care networks and local partners.

- Degree in Social Work & registration with Social Work England
- Successful completion of ASYE
- Valid UK driving licence and ability to travel for work
- Driving Licence: It is an essential requirement of the role that the post holder has a valid UK driving licence, is a car owner and can use the car for work purposes.
- Degree in Social Work & registered with Social Work England; successfully completed the UK Skills for Care ASYE programme.
- Compliance with Disclosure and Barring Service Check.
