Overview

Description: Candidate must live in/be familiar with Montgomery or Prince George's County.

Summary: Maryland Care Management, Inc. is searching for an innovative Social Worker located in Maryland who is a self-starter with a problem-solving mindset. In this role, you will work directly with MPC members to address social barriers to optimal health care while providing care management through a cooperative process of assessment, planning, and facilitation. You will also act as a member advocate when referring options and community services to meet members’ health needs.

What You’ll Do
  • Utilizes clinical judgment to assess members, prioritizing emerging issues to maintain a member-centric approach.
  • Collaborates with interdisciplinary care team to aid member health goals via case rounds and consultation.
  • Utilizes problem-solving skills to research and identify community resources and coordinate referral mechanisms.
  • Plans specific goals and actions designed to meet the member’s needs as identified in the assessment process that are action-oriented, time-specific, and cost-effective.
  • Builds strong relationships with members by aiding and advocating to help achieve health goals and through visits to the home and community, telephonic and/or video conferencing.
  • Monitors care management activities, services, and members’ responses to interventions, to track the effectiveness of the plan of care and the utilization of services.
  • Participates in outreach activities to promote knowledge of the program and its services and to coordinate program activities with outside community agencies and health care providers.
  • Hybrid position. Must be available and willing to travel up to 50% of the time to meet with members.
What We’re Looking For
  • Knowledge of community agencies and resources. (PG/MOCO)
  • Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services.
Requirements

Required Education and

Work Experience:

  • Bachelor’s degree in social work.
  • Valid Maryland state LMSW or LCSW licensure (in good standing).
  • 3 years of experience in a community setting.
  • Possession of a valid Maryland State Driver’s License.

Preferred Education And Work Experience

  • Knowledge of government Managed Care programs (Medicaid/Medicare)
  • Master’s degree in social work
