Overview

In this role you will be responsible for facilitating the inquiry and admission process of nursing home placement for patients/residents to this community from physicians, hospitals, and the general public, along with networking/promoting our services to the same. The Director of Admissions & Community Outreach will maintain budgeted occupancy goals and complies with department and community policies, procedures, and standards. This position will perform pre-admission assessments within a variety of health care and home-based settings to facilitate sound and rapid admissions decisions for the skilled nursing community.

Functions, Key Duties, & Responsibilities
  • Build relationships with case managers, discharge planners, social workers, key clinicians, surgery schedules, practice managers, physicians, specialists, and key community influencers to grow census for skilled.
  • Identify, develop, strengthen, and maintain referral sources to facilitate and drive qualified skilled referrals.
  • Develop an in-depth understanding of the campus programs and services to accurately highlight clinical capabilities with referral sources.
  • Manage all inquiries and work directly with Admissions team conducting pre-admission onsite assessments.
  • Conduct on-site visits preferably on the same day of inquiry or within 24 hours thereafter.
  • Communicate plan of care to hospital case manager at discharge.
  • Actively participate in development of marketing plans and identification of new sources for patient referrals.
  • Actively participate in special events for prospects, referral sources, families and overall community and market awareness.
  • Serve on call for admissions 24/7 and respond as appropriate.
  • Submit and participate in meetings and reports as needed to keep community leaders aware of admissions and changes in the market.
  • Communicate bed availability and discuss placement with hospitals and public.
  • Tour prospects, families, caregivers, and referral sources through community.
  • Answer telephone inquiries from families and caregivers.
  • Prepare admissions in accordance with established policy and procedures.
  • Interview residents/caregivers and obtain required information and signatures.
  • Attach pre-admission documentation to admission papers as appropriate.
  • Maintain an accurate record of available beds.
  • Collect, assemble, and check admission papers and forward documentation as needed.
  • Obtain the resident’s assigned room number and record in all necessary admission papers.
  • Explain the room rates, billing procedures, visitors/ guests’ privileges, restrictions, resident care procedures, etc., to the resident and/or guardian as appropriate.
  • Maintain various registries as directed including register for admission and discharge of residents.
  • Maintain resident/patient and community confidentiality.
  • Develop and maintain a good working rapport with inter-department personnel, as well as other departments within the community.
  • Participate in staff meetings; keep abreast of communications via posted memos and policies and procedures.
  • Keep abreast of current Medicare/Medicaid regulations governing admission/discharge requirements of health care facilities.
  • Adhere to Universal Precaution Infection Control, OSHA, Safety and Disaster policies and procedures.
  • Keep work environment and patient areas neat and free of safety hazards.
  • Understand financial position of the organization and adjust where appropriate.
  • Perform other duties as assigned.
  • Comply with all policies and procedures and any updates.
Position Requirements

Education
:
High School Diploma/GED required;
Bachelor’s in Marketing or Business related field preferred

Licenses/Certifications: N/A

Experience
: 3+ years of sales and marketing experience in healthcare required;
Senior Housing industry experience preferred

Skills & Abilities:

  • Ability to demonstrate professionalism and provide an excellent first impression of the organization/community

Travel
:
Moderate amount of local travel for marketing and special events

Compensation
: USD $85,000.00/Yr. – USD $95,000.00/Yr.
