Position Summary

The ACT Treatment Plan Coordinator is responsible for reviewing, coordinating, and advancing all ACT care plans and treatment plans. This role ensures that each client’s next steps are clearly identified and acted upon, including referrals to community resources and completion of applications for services. The coordinator works closely with the ACT team to support client progress, stability, and continuity of care.

Responsibilities
  • Treatment Plan Review & Coordination — Review all ACT care plans and treatment plans for accuracy, completeness, and timeliness; identify next steps based on client goals, needs, and treatment recommendations; track progress and follow up on assigned action items.
Referrals & Applications
  • Refer clients to appropriate community resources, programs, and services
  • Complete and submit applications for benefits, housing, medical, social, or supportive services
  • Communicate with external agencies to support referrals and application approvals
Collaboration & Communication
  • Work closely with ACT team members, including clinicians, case managers, and providers
  • Participate in team meetings and care coordination discussions
  • Ensure treatment plans align with clinical recommendations and service requirements
Documentation & Compliance
  • Maintain accurate and timely documentation in client records
  • Ensure care plans meet regulatory, organizational, and funding requirements
  • Support audits, reviews, and quality assurance processes
Qualifications

Required

  • Bachelor's degree in social work (BSW), Human Services, Psychology, or a related field
  • Experience in care coordination, case management, or social services
  • Strong organizational skills and attention to detail
  • Ability to manage multiple plans, deadlines, and follow-ups
  • Knowledge of community resources, benefits, and service systems

Preferred

  • Master's degree in social work (MSW)
  • Experience working within an ACT (Assertive Community Treatment) model
  • Familiarity with behavioral health, mental health, or community-based services
Skills & Competencies
  • Strong written and verbal communication skills
  • Ability to problem-solve and identify practical next steps
  • Client-centered and team-oriented approach
  • Ability to work independently and collaboratively
  • Comfortable completing applications and navigating service systems
