Recruitment Services are seeking a Personal Advisor Leaving Care for our client based in Lincoln. Hybrid working, the client requires 2 days working from the office.
As a Leaving Care Worker, you will play a vital role in supporting young people aged 16–25 who are transitioning from care to independent living. You will act as a Lead Professional, building strong relationships and delivering tailored support plans that empower care leavers to achieve their goals.
DUTIES &
RESPONSIBILITIES INCLUDE:
Caseload Management:
Hold responsibility for a defined caseload of care leavers, ensuring timely assessments, planning, and interventions.
Pathway Planning:
Develop and review aspirational Pathway Plans covering accommodation, education, employment, health, and financial independence.
Practical Support:
Assist with budgeting, bills, tenancy management, shopping, and life-skills training.
Advocacy & Coordination:
Work collaboratively with multi-agency partners (housing, education, health, benefits) to secure positive outcomes.
Emotional Support:
Provide trauma-informed, relationship-based support to promote resilience and wellbeing.
Safeguarding:
Identify and manage risks, ensuring compliance with statutory duties and safeguarding protocols.
Record Keeping:
Maintain accurate case notes and reports in line with local authority standards.
Duty Cover:
Participate in duty rota to respond to urgent needs and crisis situations.
ESSENTIAL REQUIREMENTS INCLUDE:
Qualifications:
NVQ Level 3 in Health & Social Care (or equivalent).
Experience:
Minimum 2 years working with young people in care or leaving care services.
Knowledge:
Children Act 1989, Care Leaver entitlements, safeguarding frameworks.
IT proficiency (case management systems, MS Office).
Full UK driving licence and access to a vehicle.
Enhanced DBS (Children & Adults)
