Overview
An excellent opportunity for a full time Band 6 CPN to join the Lincoln Locality Mental Health Team. For current LPFT staff this can be offered as a Secondment for 12 months; external applicants will have a fixed term for 12 months.
Responsibilities- Provide high quality person-centred care to patients, families and carers.
- Act as Lead Healthcare Professional, undertaking comprehensive evidenced-based assessments.
- Apply sound knowledge of psycho-social interventions and support service users to work towards recovery goals, build self-resilience and promote independent living.
- Work within a friendly and supportive Multi-Disciplinary Team (including Community Mental Health Nurses, Recovery Practitioners, Occupational Therapists, Nursing Associates, Community Support Workers, Psychiatrists, Psychologists and Admin staff) and alongside Neighbourhood Teams, Integrated Placement Based Teams, Adult Social Care, GP surgeries and other partner organisations to achieve the best outcomes.
- Demonstrate aRecovery-focused way of working embedded in team culture, empowering service users and instilling hope.
- Contribute to service development within a complex and changing service; demonstrate excellent communication and leadership skills.
- Be flexible with a proactive, can-do approach; able to work under pressure and adapt to changes, often at short notice.
- Receive robust clinical and managerial supervision regularly to support role performance; participate in training and professional development to stay up-to-date with evidence-based practice.
- Be able to travel independently in the community to deliver services, adopting a hybrid approach to working.
- Qualified RMN applying for a Band 6 CPN role in a recovery-focused service for adults with severe and enduring mental ill health.
- Ability to engage in service development within a complex service environment; strong communication and leadership capabilities.
- Flexible, resilient, capable of working under pressure and managing competing demands.
- Proactive team player with a positive attitude and commitment to collaborative working.
- Own transport or ability to travel as required to deliver community-based services.
