Social Worker Level 2 – West Community Wellbeing Team

What you’ll do- Work closely with adults and families to provide tailored support and care planning in a strengths based and person centred way, embracing what community assets and strengths.
- Champion independence, dignity, and wellbeing in every interaction
- Collaborate with health professionals, community partners, and colleagues to deliver joined-up services
- Bring compassion, resilience, and creativity to complex situations
- Help develop our workforce through supervision of up to 4 Community Care Officers

Employer:
