Position: PCN High Intensity Use (HIU) Lead

We are excited to recruit a full-time HIULead to support the launch of our new High Intensity Use (HIU) Service apioneering initiative within our Primary Care Network (PCN), made up of four GP practices working in close partnership with neighbourhood teams.

Main duties of the job

The HIU Service supports individuals who frequently attend A&E or use emergency services more than expected. Many face complex challenges including poor physical or mental health, housing instability, loneliness, or substance misuse. This service takes a person-centred approach, building trusted relationships and offering long-term, practical support to help people access the right care and improve their quality of life.

About us

APEX PCN is located in West Lincoln and incorporates 4 practices Birchwood Medical Practice, Boultham Park Medical Centre, Richmond Medical Centre and Woodlands Medical Practice. We have a population of 46,000 with a predominantly younger demographic. The PCN is supported by a strong ARRS team. This includes pharmacists- engaged in supporting practices in their daily workload while also focussing on the DES requirements.

We have an effective FCP team supporting practices for MSK care and a well-developed EHCH and Frailty MDT to support care homes. Our practices work collaboratively to achieve our IIF targets and PCN DES requirements. We are now looking at forming strategic partnerships with our other Health and Care colleagues to effectively address the health needs of our population.

Job responsibilities

This is a non-clinical role focused on listening, understanding, and empowering. As a HIU Lead, you will:

  • Work directly with individuals to explore whats really going on in their lives
  • Build trust and offer guidance tailored to each persons needs
  • Connect people with appropriate services including health, housing, social care, and community support
  • Take a collaborative, person-centred approach to reduce avoidable emergency service use
