Job Summary

APEX PCN serves a population of nearly 47,000 patients. The PCN has been selected to take part in an NHS England Pilot to improve patient access. The aim of the hub is to absorb same‑day patient demand, enabling practices to focus on population health and chronic disease management. We are seeking an enthusiastic team member to join this fixed‑term, exciting opportunity.

About Us

APEX PCN is located in West Lincoln and incorporates four practices:
Birchwood Medical Practice, Boultham Park Medical Centre, Richmond Medical Centre and Woodlands Medical Practice. The PCN is supported by a strong ARRS team, pharmacists, an FCP team, and a well‑developed EHCH and Frailty MDT.

Responsibilities

• Provide care for patients from initial history taking, clinical assessment, diagnosis, treatment and evaluation.
• Use safe, clinical decision‑making and expert care, including assessment and diagnostic skills.
• Manage undifferentiated conditions, identify red flags and take appropriate action.
• Use complex decision‑making to inform diagnosis, investigation and management within a broad scope of practice.
• Actively take a personalised and population‑centred care approach, enabling shared decision‑making.
• Complete the relevant training to provide multi‑professional clinical practice.
• Utilise sensitive communication styles to ensure patients are fully informed and consent to treatment.
• Communicate effectively with patients, carers and stakeholders, anticipating barriers to communication.
• Maintain effective communication within the urgent care service and with GP practices.
• Act as an advocate for patients and colleagues.
• Maintain the security of sensitive personal information and comply with Information Governance policies.

Candidate Qualifications

• Registered first‑level nurse MSc or equivalent.
• Independent prescribing qualification.
• Relevant nursing or paramedic degree.
• System One experience.
• Clinical leadership skills.
• Working autonomously in an advanced nursing role.
• Nurse prescribing experience.
• Experience working within a primary care or general practice.
• Experience in clinical assessment and management of a wide range of minor illness/injury in patients of all ages.

Desirable Skills

• Nurse‑led triage.
• Working with community development initiatives.
• Experience in case management.
• Health needs assessment.

Additional Requirements

• Meet NMC advanced practice requirements.
• Subject to DBS check; subject to Rehabilitation of Offenders Act.
• Current UK professional registration.
