Pharmacy Technician
Askilled, experienced and enthusiastic Pharmacy Technician, to work with the Complex Care Team, at Lincoln Healthcare Partnership Primary Care Network (PCN). An exciting new team with a goal to provide, joined up, pro-active care for thosewith complex health and social conditions, requiring multiple services tosupport them in their place of residence. Working along side the High intensity pilot.
The successful candidate will work with the team to ensure safe, accurate, timely supply of prescribed medication, being part of a multidisciplinary healthcare team.
The Pharmacy Technician plays an important role, complementing clinical pharmacists, community pharmacists and other members of the Primary Care Network multi- disciplinary team. Pharmacy technicians core role responsibilities cover clinical, and technical and administrative categories.
Main duties of the jobThe purpose of the role is to lead improvements to maximise safe, cost-effective best practice in prescribing to improve the quality of patient care. The post holder will help patients to get the best from their medicines by switching medications to agreed and approved protocols, improving repeat prescribing processes in General Practice, including promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines.
About usLincoln Healthcare Partnership (LHP) PCN is made up of two practices (Brayford Medical Practice and Heart of Lincoln Medical Group) based in central Lincoln with apatient population of just under 40,000 people.
Job responsibilitiesThe following are the core responsibilities of the Pharmacy Technician. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
- a. Undertake patient facing and patient supporting roles to ensure effective medicines use,through shared decision-making conversations with patients.
- b. Carryout medicines optimisation tasks including effective medicine administration(e.g. checking inhaler technique), supporting medication reviews and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.
- c. As determined by the PCN, support medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings, linking with local community pharmacies
- d. Supportin Structured Medication Reviews (SMR)
- e. Provide specialist expertise, where competent, to address both the public health andsocial care needs of patients, including lifestyle advice, service information,and help in tackling local health inequalities.
- f. Takea central role in the clinical aspects of shared care protocols and liasingwith Clinical Pharmacists complex patients.
- g. Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing locally.
- h. Assistin the delivery of medicines optimisation and management incentive schemes and patient safety audits
- i. Support the PCN multi-disciplinary team to ensure efficient medicines optimisation processes are being followed.
- j. Implement efficient ordering and return processes and reducing medication wastage.
- k. Provide training and support on the legal, safe and secure handling of medicines,including the implementation of the Electronic Prescription Service (EPS).
- l. Promotion of Electronic Repeat Dispensing (eRD) and online ordering
- m. Develop relationships with other pharmacy professionals and members of themulti-disciplinary team to support integration across health and social care including primary care, community pharmacy, secondary care and mental health.
- n. Support practice reception teams in streaming general prescription requests, so as toallow GPs and clinical pharmacists to review the more clinically complex requests.
- o. Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings.
- p. Support the PCN to deliver on QIPP agenda, QOF and locally commissioned enhanced services
- q. Supporting quality improvement measures and contributing to the Quality and Outcomes Framework and enhanced services;
- r. Support the PCN in reviewing and developing practice policies for CQC requirements
- s. Developa culture that promotes equality and values diversity. The post-holder must beaware of and committed to the Equality and Diversity policies of the appointing
