Job Summary
The Registered Nurse Care Team Leader (Case Manager) provides professional nursing services in accordance with physicians’ orders or plan of care to patients age 18 and older for a wide range of diagnoses. The RN, CTL determines nursing needs for an assigned group of patients/residents, initiates the plan, and evaluates its effectiveness. The role supports the Village mission and values, completes required training hours, and may participate in travel to patient homes within the Kansas City Metropolitan Area and Cass County.
Essential Job Functions
- Provide timely care to ensure comfort and well‑being of patients by assessing conditions, processing physicians’ orders, administering treatments and drugs.
- Coordinate nursing care and educate patients/residents and families.
- Delegate nursing tasks to staff, monitor delivery of care, and document progress toward goals.
- Report patient changes and needs to appropriate staff.
- Drive as required to patient homes.
- Adhere to safety regulations and maintain professional conduct.
- Work effectively with colleagues and patients within the workplace.
Essential Qualifications
- Education: Associate’s Degree in Nursing (ADN) or higher.
- Licenses &
Certifications:
Registered Nurse licensed in Missouri and Kansas; current Basic Life Support (BLS) certification (or ability to obtain prior to patient contact); valid driver’s license and proof of insurance. - Knowledge &
Skills:
Proficiency with Microsoft Office (Word, Excel, Outlook); ability to manage multiple tasks; autonomous work in a home‑health setting; effective written and verbal communication; basic math skills and problem‑solving ability.
Special Working Conditions
- Work inside and outside environments; exposure to infectious diseases.
- Handle patients with mental/physical challenges.
- Drive between patient locations; possible on‑call duties.
Physical and Mental Demands
- Occasional lifting up to 50 lbs; frequent lifting up to 20 lbs; constant lifting up to 10 lbs.
- Visual acuity for close‑distance work and computer use.
- Auditory acuity for normal conversation.
- Repeated bending, stooping, kneeling, crouching, reaching, standing, walking, pushing, pulling, typing, and repetitive wrist motions.
Supervision Received
- Receives daily supervision from the Clinical Manager.
Supervision Given
May provide direction to caregivers on a daily basis.
