Hospice RN Case Manager –
John Knox Village
Full‑time, salaried and exempt with a competitive pay and benefits package.
Responsible for delivering professional nursing care in accordance with physicians’ orders and the hospice care plan. Responsible for collaboration and oversight of the interdisciplinary team (IDT) and support of the Village’s mission, values, and service/hospitality (PRIDE) expectations.
- Provide home or facility visits for patient assessment, treatment, and support, including pronouncement of patient death, referral management, patient admission, and crisis management.
- Plan comprehensive services for patients and families, develop and revise nursing care plans, and set goals in collaboration with the physician.
- Delegate appropriate nursing responsibilities, psychosocial or emotional needs to team members and ensure delivery of care.
- Report changes or needs in patient condition to the relevant team members.
- Perform phone triage of patient‑related calls using clinical knowledge and judgment, resolving problems at the point of contact whenever possible.
- Make sound decision making and set priorities.
- Demonstrate excellent customer‑care skills and respectful communication to all internal and external customers.
- Maintain up‑to‑date knowledge of hospice and community programs and services.
- Apply knowledge of Medicare and Medicaid regulations.
- Complete documentation and care‑coordination notes promptly; all documentation should be synched by the end of each day.
- Assist in delivering medications and supplies as needed, including during business hours, weekends, and holidays.
- Act as a preceptor by orienting new nursing staff and students as directed.
- Attend staff and interdisciplinary meetings, including education services as required by agency policies.
- Participate in committee meetings and performance‑improvement activities (QAPI).
- Perform care consistent with Village Hospice standards of practice.
- Comply with JKV and agency policies and procedures.
- Adhere to all safety regulations and workplace conduct standards.
- Handle stress well and maintain positive relationships with workplace colleagues and external customers.
Primary responsibilities described above represent the core duties of the position. Other duties may be assigned by the supervisor as warranted by business needs.
Essential Qualifications Education- BSN preferred; required completion of an accredited School of Nursing.
- Registered Nurse licensed by the Missouri State Board of Nursing or a multi‑state compact license that allows practice in Missouri.
- Current Basic Life Support (BLS) certification or ability to obtain prior to direct patient contact.
- Valid driver’s license and proof of insurance required.
- Certified Hospice Palliative Nurse (CHPN) preferred.
- Basic proficiency with clinical software and Microsoft Word and Outlook.
- Experience triaging patient‑related calls preferred.
- Ability to work in a field setting and make sound nursing judgments.
- Ability to assess patient needs and formulate individualized patient‑care plans.
- Knowledge of physical, psychosocial and spiritual needs of terminally ill patients and caregivers.
- Ability to manage multiple tasks and responsibilities autonomously.
- Ability to read, analyze, and interpret professional journals, technical procedures, and government regulations.
- Ability to define problems, collect data, establish facts, and draw valid conclusions.
- Access to patient residences in inside and outside environmental conditions.
- Exposure to infectious diseases.
- Function around individuals with challenged mental capacities.
- Required to drive between locations for services.
- Potential “on‑call” responsibilities.
- All associates may be called upon to assist other departments in a declared emergency situation.
- Maximum occasional lift of 50 lbs from floor to waist level; maximum occasional lift of 10 lbs from floor to shoulder; maximum occasional carry of 5 lbs for 100 ft; maximum occasional push/pull horizontal of 40 lbs at 36 in height for 100 ft; maximum frequent push/pull horizontal of 10 lbs at 46 in for 100 ft; maximum occasional grip force of 40 lbs;
maximum occasional lateral pinch force of 15 lbs; maximum occasional climb of 8 in; frequent forward reach; frequent bend; frequent fine motor coordination; occasional foot coordination; occasional balance; frequent sitting; continuous standing; and continuous walking. PDC level: medium.
Receives daily supervision. May provide direction to caregivers on a daily basis.
