Shifts Available: M-F, 8am-4:30pm, On-Call Rotation
Responsibilities- Ensure that the unit operates efficiently, meets its goals, manages resources effectively, and coordinates with other units or departments as needed.
- Lead staff in delivering exceptional care to residents and support to their family members.
- Maintain resident records according to State and Federal guidelines.
- Help to maintain and restore health of resident.
- Act as a main point of contact between residents and their families.
- Address and assist in preventing concerns from family members.
- Assist in developing, implementing, and making adjustments to patient care plans as needed.
- Supervise, counsel, and provide discipline, when necessary, to nurse staff to ensure the highest quality of care possible.
- Ensure the unit meets federal and state regulatory standards, as well as facility policies and procedures.
- Collaborate with the Director of Nursing, Nurse Practitioners and Physicians to plan and maintain resident care.
- Complete admission and discharge assessments.
- Oversee and audit changes made to care plans.
- Deliver hands-on care and medicine distribution as needed.
- Audit and complete charting, evaluations, medical forms, and other administrative tasks.
- Identify procedural problems and work with the Director of Nursing to resolve issues.
- Evaluate, prioritize and meet the care needs of each resident.
- Participate in Quality Assurance, Safety, PIPs and other committees as required.
- Provide a safe working environment for staff members, promptly reporting any accidents and implementing corrective and preventive action.
- Participate in daily morning meetings.
- Provide education and coaching to nursing staff as needed to improve patient/resident outcomes.
- Provide clinical backup to floors as needed.
- Take weekend on-call according to the assigned on-call rotation schedule (if applicable).
- Complete chart audits for Medicaid patients monthly.
- Develop and maintain the nursing policies and procedures as needed.
- Additional duties as assigned.
- Registered Nurse or Licensed Practical Nurse with license in good standing in the state of Nebraska or any other Compact/Multi-state license.
- Active CPR/BLS Required.
- Demonstrate experience working with older adults in a resident-centered care role.
- Excellent interpersonal and communication skills.
- Ability to work within a multidisciplinary team.
- Demonstrate maturity, flexibility and self-direction.
- Ability to communicate openly, honestly and respectfully with others.
- Ability to maintain a customer focus, treating all residents with respect and integrity.
- Ability to read, speak, and understand the English language.
