Now Hiring – LPN – Lee’s Summit, MO Area
- Completing an initial assessment of patient and family to determine hospice care needs
- Regularly re-evaluating the patient’s nursing needs
- Using assessment data to determine care plan needs
- Collaborating with the healthcare team, patient, and family to direct coordinated care
- Counseling the patient and family on end‑of‑life needs
- Initiating appropriate preventative and palliative nursing procedures
- Communicating with the physician regarding patient needs and reporting changes in the patient’s condition
- Supervising the Hospice Aides
Associate’s Degree in Nursing
Professional Experience- Must have a State Certified RN license
- 2 years’ experience as a professional nurse, hospice experience preferred
- Must have a valid Driver’s license and reliable transportation
- Must be willing to travel
- Strong verbal and written communication skills
