Home Health and Hospice RN Case Manager - Full-Time | Omaha, NE
Aegis Healthcare is expanding our Home Health and Hospice Nursing Team and is looking for a compassionate and experienced Home Health and Hospice RN Case Manager who is dedicated to making healthcare better for everyone.
Position Details- Compensation: Our RNs average $85K/year
- Employment Status: Full-Time
- Schedule: Flexible Scheduling Monday-Friday, 8 AM - 5 PM

Your Role as a Hospice RN Case ManagerAs an RN Case Manager, you will play a critical role in providing exceptional home health and hospice care. Your responsibilities will include:
- Comprehensive Patient Assessments - Evaluating the physical, social, and psychological needs of patients.
- Collaborative Care Planning - Engaging patients, families, and interdisciplinary teams to develop holistic care plans.
- Patient-Centered Approach - Providing compassionate support to enhance comfort and quality of life.
- Resource Coordination - Managing patient care resources and referrals efficiently.
- Accurate Documentation - Maintaining meticulous patient records to ensure seamless care.
- Current RN License in Nebraska
- Home Health and Hospice experience preferred
- Ability to assist with patient transfers using full range of movement
- Strong verbal and written communication skills
- Excellent time management and organizational abilities
- Valid Driver's License and Reliable Transportation
- Proof of Auto Insurance and Registration
- Current CPR Certification (we can help obtain)
- Negative TB Test (we can provide testing)
- HEP B Record
