ReDiscover is seeking a Community Support Specialist to add to our Connections Program!

ReDiscover has been known for over 50 years as a non-profit community mental health center in southeastern Jackson County, Missouri with a mission of delivering efficient and cost-effective treatment for the people we serve. What We Do

ReDiscover's Connections Program is tied to Missouri's Community Psychiatric Rehabilitation Program (CPRP). This is a case management focused program that provides individuals with support to live an independent life in the community. Through a variety of multidisciplinary services that clients can choose from including medication management, therapy, group therapy, health and wellness support, employment services and transportation support, clients can develop skills, access resources and learn ways to manage their emotional and physical health.

This position will assist deaf clients enrolled in ReDiscover's CPRP programs.

At ReDiscover, while every day is different, a typical day may include the following roles and responsibilities:

  • Maintain and interact with a caseload of clients with diverse characteristics either in-person or over the phone.
  • Facilitate each client’s achievement of long range outcomes of symptom management, employment and independent living based upon the needs of the client.
  • Engage difficult to serve clients by investigating all potential resources in order to gain access and make connections.
  • Help remove barriers for clients, which may include helping clients participate in community or social service programs, referring clients to educational or working programs and/or assisting with transportation barriers as identified in their treatment plan.
  • Collect and maintain information about clients and document interactions within ReDiscover’s designated electronic medical record.
  • Communicate and collaborate with an internal multi-disciplinary team to address various aspects of wellness and ensure positive communication regarding the coordination of client care.
We are focused on your development and will spend time helping you continue to grow as a Community Support Specialist. However, here are some things that you will need prior to starting with ReDiscover:

  • Availability to work 20 hours a week with a flexible schedule.
  • 4-year degree or 4 years of equivalent experience in lieu of degree.
  • A valid automobile driver’s license, appropriate automobile insurance and a safe, reliable, registered automobile available for use during work.
  • Fluency in American Sign Language; deaf candidates are preferred and are highly encouraged to apply.
  • Familiarity with deaf culture.
  • At least two (2) years clinical experience in a direct service provision to children and adolescents in a mental health setting preferred.
  • Working knowledge of mental illness and substance use disorders.
