This position provides direct human services support using a range of rehabilitation and supportive modalities to assist clients in achieving stability and recovery. The role focuses on working with justice-involved individuals who experience serious mental illness, supporting their engagement in treatment, services, and community resources. Individuals in this position are integral members of Lancaster County’s Mental Health Diversion and Problem-Solving Program team, collaborating with justice system partners and behavioral health providers to promote positive outcomes and reduce recidivism.
Responsibilities- Providing overall case management services to an assigned group of clients.
- Planning, coordinating, evaluating and delivering prevocational and rehabilitation services.
- Skill building and crisis intervention.
- Performing initial rehabilitation and support evaluations and presenting findings for staff review.
- Maintaining appropriate client records reflecting services provided and client progress.
- Transporting clients to and from appointments.
The primary emphasis of this class is centered upon providing rehabilitation and supportive mental health services requiring the use of fundamental educational or rehabilitative techniques to satisfy client’s needs.
QualificationsGraduation from a senior high school supplemented by completion of an associate degree program in human services or mental health field emphasizing techniques utilized in a mental health program or any equivalent combination of training and experience that provides the desirable knowledge, abilities and skills.
