Job Summary
Responsible for evaluation and development of treatment plans to restore, maintain or prevent the decline of disabilities, injuries, and diseases in accordance with the orders of the referring physician. Supportive of the Village’s mission, values, and service/hospitality (PRIDE) expectations. Completes training as required by policy.

Essential Job Functions- Visits Home Health patients to perform assessments and establish a plan of care. Treats patients or directs treatment referred for speech therapy services by written order of the patient’s physician.
- Establishes appropriate treatment plan based on problems identified in the evaluation.
- Communicates with physician regarding the evaluation results and establishes a plan of care according to the physician’s orders.
- Communicates with the patients and responsible parties concerning the treatment program.
- Teaches patient and families regarding therapy plan of care.
- Evaluates effects of therapy treatments by observing, noting and evaluating patient’s progress; provides medically necessary treatment plan and modifications.
- Maintains properly documented records for each patient in compliance with regulations and policies.
- Initiates and participates in case conferences as pertains to patient goals and care delivered.
- Participates in performance improvement activities upon request.
- Protects patients by adhering to treatment location infection control policies and protocols and keeping information confidential per company and federal requirements.
- Adheres to all safety regulations and requirements.
- Handles stress well and collaborates with colleagues and customers.
- Maintains acceptable standards of workplace conduct.
The responsibilities described above represent the primary responsibilities of the job. Other responsibilities may be assigned by the supervisor as warranted by business needs. The incumbent is expected to perform all assigned responsibilities.
EducationMasters of Science degree in Speech-Language Pathology from an accredited school of speech-language pathology.
Licenses / Certifications- Licensed Speech Therapist in MO and KS required; must obtain license within first 90 days of employment.
- Current valid driver’s license and proof of insurance.
- Current CPR Certification or must obtain within first 90 days of employment.
- Current valid Certificate of Clinical Competence in Speech Pathology (CCC-SLP) granted by the American Speech-Language-Hearing Association.
- Basic level knowledge of Microsoft operating system and Microsoft Word, Excel and Outlook.
- Ability to travel to patient homes within designated geographic area.
- Ability to manage multiple tasks and responsibilities simultaneously.
- Good problem‑solving skills and ability to work autonomously.
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
- Ability to write reports, business correspondence, and procedure manuals.
- Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
- Ability to perform basic arithmetic calculations and apply them to case documentation.
- Ability to define problems, collect data, establish facts, and draw valid conclusions; and deal with a variety of abstract and concrete variables.
- One year experience in Speech Therapy preferred.
- Exposure to indoor environmental conditions but not necessarily to temperature changes.
- Exposure to infectious diseases.
- Required to work around individuals with challenged mental capacities.
- Required to drive from one location to another if necessary for services provided.
- May have on‑call responsibilities.
- Potential exposure to outdoor elements while accessing a patient’s residence.
- May be called upon to assist other departments in a declared emergency situation.
- Medium work – exerting up to 50 lbs of force occasionally, and/or up to 20 lbs of force frequently, and/or up to 10 lbs of force constantly to move objects.
- Requires adequate visual acuity with or without correction.
- Frequent bending, kneeling, climbing stairs, and reaching in various directions.
- Frequent walking, especially for long distances or moving work sites.
- Requires manual dexterity for picking, pinching, typing, and handling objects.
- Requires clear verbal communication and hearing at normal speaking levels.
Reports to Administrator Home Health. Direction given is none.
