• MD or DO degree from a recognized medical institution.
• Completion of an accredited Family Medicine residency program.

• Certification by the Board of Family Medicine.
• Eligible for (or currently possess) an active Nebraska medical license.
• BLS and ACLS certifications are preferred but not mandatory.
