Registered Nurse; RN - Home Health

Overview

Graham Healthcare Group is seeking a Registered Nurse (RN) – Home Health for a nursing position in Lincoln, Michigan.

Employer:

Residential Home Health and Hospice (Residential). This role supports patients in Lincoln and surrounding areas and is eligible for a $10,000 first year success bonus based on achieving key milestones during the first year.

• Lead and collaborate with a team to provide management and delivery of patient care plans.
• Deliver patient care in a home health setting as a registered nurse case manager.
• Maintain timely and accurate charting to meet critical deadlines.
• Utilize tablets, mobile devices, and EMR software to support care delivery.

• RN license in the state of the work location.
• Graduate from an approved school of nursing (RN pathway).
• One year of RN experience with clinical competence to deliver quality patient care.
• Current driver’s license and ability to spend approximately 20% of time driving to/from patient locations.
• Commitment to flexible scheduling and the ability to self-manage time.
• Excellent use of technology and comfort with EMR systems, tablets, and mobile devices.

• Duration:
  Ongoing; 40 hours per week; 8-hour shifts.
