Connections Plus Healthcare + Hospice began in 1983 and was known as Rockbridge Area Hospice. In 2023, Rockbridge Area Hospice merged with Mountain Regional Hospice, increasing their reach and resources while maintaining excellent care and staff.

It has grown from supporting one patient with bake sales to serving the broader areas of Lexington, Buena Vista, Rockbridge County, Clifton Forge, Covington, Alleghany, and Bath County. With a new mission statement reflecting expanded services, Connections Plus Healthcare + Hospice provides compassionate care for individuals with serious illnesses. • Provide comprehensive nursing care in homes and facilities to patients in accordance with individualized care plans.
• Manage pain and symptoms with clinical excellence
• Maintain accurate medical documentation in compliance with HIPAA regulations.
• Educate and support families regarding disease processes, treatment options, and end-of-life care.
• Collaborate with physicians, social workers, and care teams
• Participate in case management activities, including discharge planning and coordination of services.

• Current RN license in Virginia
• Hospice, palliative, or home-health experience preferred
• Strong assessment skills and compassionate presence
• Comfort with EMR systems and medication administration
• Someone who values collaboration and mission-driven work
