Summary
The Home Infusion Nurse will be responsible for providing high-quality, in-home infusion therapy to patients. This role requires a Registered Nurse (RN) license and the ability to deliver exceptional patient care while managing various infusion treatments in a home setting. The RN utilizes physicians, community resources, and the patient's support system ensuring that all care delivered meets agency standards and regulatory requirements.
Job Description:
Baptist Health is looking for a Home Infusion RN to join our team at Baptist Health Lexington Home Care, 2100 Nicholasville Rd, Lexington, KY!
4 days/32 hours per week, 9am - 5:30pm.
Rotate on call holidays (Summer/Winter) with other RNs.
Rotate weekend call with other RNs.
- Collaborate with the healthcare team, including physicians, pharmacists, and case managers, to develop and implement individualized care plans.
- Ensure timely communication and documentation of patient status and care provided.
- Educate patients and their families on infusion procedures, medication administration, and how to manage potential side effects.
- Provide guidance on maintaining a sterile environment and proper use of infusion equipment.
- Conduct thorough assessments of patients' health status, including vital signs, response to therapy, and overall progress.
- Identify and report any adverse reactions or complications to the supervising physician.
- Manage and maintain infusion supplies, equipment, and medications, ensuring that all are available and properly stored.
- Coordinate with the pharmacy and suppliers to replenish inventory as needed.
- Maintain accurate and up-to-date patient records, including treatment plans, progress notes, and medication logs.
- Associates degree required, BSN degree preferred.
- Graduate of an accredited nursing program.
- 2 years of job-related experience.
- Must have valid KY RN license.
- BLS issued by American Heart Association or must obtain within 90 days of hire.
- Infusion Nursing Certification (CRNI) is a plus.
- Must have and maintain a valid driver's license.
- Experience with central lines, PICC lines, ports and peripheral IVs preferred.
