Job SummaryThe Patient Transfer Coordinator coordinates placement of all admissions and transfers to UK Chandler / Good Samaritan according to clinical standards. Ensures patients are admitted to the appropriate bed using current criteria. Provides total, comprehensive, continuous patient‑centered care, including assessment, documentation, planning, implementation, teaching, developing, and evaluating comprehensive nursing care plans; delivers direct patient care; ensures patient satisfaction; adheres to University policies, procedures, and federal, state, local regulations.
Supports collaborative efforts among nursing staff and physicians to enhance communication and expected outcomes.
- Coordinate placement of admissions and transfers to UK Chandler / Good Samaritan.
- Apply current admission criteria to determine appropriate bed placement.
- Assess, document, plan, implement, teach, develop, and evaluate comprehensive nursing care plans.
- Deliver direct patient care and ensure patient satisfaction.
- Adhere to University policies, procedures, and federal, state, local regulations.
- Collaborate with nursing staff and physicians to improve communication and patient outcomes.
- ICU experience preferred.
- Strong communication and organizational skills.
- Ability to work extended periods and in shift hours.
BSN preferred.
