This position requires occasionally transporting objects up to 25 lbs, sitting at a workstation for extended periods of time, repetitive motion, and occasional job-related travel; must have reliable transportation to and from events/locations.
ShiftMonday-Friday, 8am-4:30pm with some weekends and holidays per department need.
Job SummaryThe Affiliate Nurse Liaison will work with patients, Gill Affiliate Network (GAN) members and clinical care teams throughout the Gill Heart & Vascular Institute to help patients navigate their way to timely, quality care and to assist in overcoming barriers to care.
The nurse liaison will ensure that new referrals are scheduled with the appropriate Gill Heart & Vascular Institute provider and that all documentation/records are available to the provider on the day of the initial visit. They will assist patients by providing education and emotional support before and after their visit to UK Healthcare and coordinating communication between the UK Gill Heart & Vascular Institute and the referring GAN provider.
The Affiliate Nurse Liaison will assist patients from initial diagnosis through diagnostic testing, treatment, and follow-up, as needed. The goal of the Nurse Liaison is to assist GAN hospitals with timely referral for their patients and allow for increased patient and referring provider satisfaction.
