Job SummaryThe College of Nursing has contracted with the Cabinet for Health and Family Services, Department for Medicaid Services (DMS) to provide a team of 10-12 licensed Registered Nurses and 1 Nurse Manager to support comprehensive assessments using the Home and Community Based (HCB) waiver assessment tool for frail, elderly, and physically disabled populations. The Clinical Program Manager will lead and guide the team, developing educational materials for professional staff, families, and individuals seeking HCB services, and oversee data review to ensure accurate determinations of care needs.
The role is based in Louisville, Kentucky, requiring evening hours, heavy in-state travel, and occasional out-of-state travel.
Knowledge of physical and psychological development; excellent analytical and decision‑making skills; strong communication ability with physically disabled and elderly individuals; willingness to travel throughout Kentucky regularly and occasional out‑of‑state overnight trips.
Supervisory ResponsibilitiesYes – leading a team of licensed nurses.
Preferred Education/ExperienceExperience working with the elderly and physically disabled preferred.
