As a BCN Hospice Team member, you will be part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness, but instead choose care to improve their comfort and quality of life. Our service allows team members to focus on one patient at a time, providing care where the patient calls home, which could be a house, apartment, nursing home, assisted living facility, or hospital.

For more complex cases or higher care needs, care can be provided at our Hospice Care Centers.

Social Worker (SW) - PRN

Location: Lexington, KY

Schedule: PRN (as needed)

As a BCN Hospice Team member, you will provide care and support to patients and families, focusing on comfort and quality of life. You will assess psychosocial problems, define treatment goals, and work to alleviate these issues as part of an interdisciplinary team. You will coordinate patient care with the primary nurse under supervision.

• Bachelor’s or Master’s degree in Social Work accredited by CSWE; licensed as LSW, CSW, or LCSW in Kentucky
• Minimum two years of social service experience in healthcare or social services; interdisciplinary team experience preferred
• Knowledge of death, dying, and bereavement processes
• Understanding of psychosocial and legal aspects of terminal illness
• Knowledge of healthcare team dynamics
• Valid Kentucky driver’s license and reliable automobile
• Ability to handle confidential matters with discretion
• Ability to make home visits in various settings
• Good communication skills and flexibility
• Compliance with health and vaccination policies
• Personal auto insurance in accordance with state/organization requirements

Salary: $26.89 - $42.96 per hour
