Rate: $2,340/40-hours

Requirements:

Active and unrestricted Registered Nurse (RN) license in the practicing state.

Experience in post-acute, sub-acute rehab, skilled nursing, or long-term care preferred.

Strong assessment, clinical judgment, and critical thinking skills.

Ability to manage multiple residents and prioritize care effectively.

Knowledge of state/federal LTC regulations and infection control standards.

Excellent communication and documentation skills.

BLS certification (ACLS may be required depending on facility).

Compassionate, patient-centered approach to care.

Responsibilities:

Conduct initial and ongoing nursing assessments and document resident conditions accurately.

Develop, implement, and update individualized care plans in collaboration with the care team.

Administer medications and treatments safely and according to physician orders and facility protocols.

Monitor residents for changes in condition and initiate appropriate interventions.

Provide wound care, IV therapy, and post-acute rehabilitation support as needed.

Coordinate with physicians, therapists, CNAs, and other healthcare professionals to ensure continuity of care.

Educate residents and families about care plans, medications, and health management.

Maintain accurate, timely, and compliant charting and documentation.

Assist with admissions, transfers, discharges, and resident assessments (MDS input as applicable).

Ensure infection control practices and regulatory compliance at all times.

Respond to emergencies and perform life-saving measures when necessary.

Support and guide CNAs and other support staff to uphold quality care standards.
