Licensed Social Worker

Social Worker Responsibilities You'll Love:

• Provide counseling and emotional support to residents and their families

• Assess social and psychological needs of residents and develop care plans

• Advocate for residents and ensure they receive necessary services and resources

• Assist with discharge planning and community referrals

• Coordinate with interdisciplinary team members to provide holistic care

• Maintain accurate and confidential documentation as required

• Active Social Worker license in Delaware

• Experience in long-term care or healthcare settings preferred

• Strong communication, interpersonal, and problem-solving skills

• Compassionate and client-focused approach
