Overview

The Community Outreach Population Health Nurse Practitioner will provide primary health care and behavioral health services as part of a multidisciplinary team that includes Behavioral Health and Community Outreach RNs, Peers, Community Health Workers, and drivers. The NP will work within the scope and credentials approved by Beebe Healthcare, including licensed NP and DEA license.

In this role, the NP works on our mobile health clinic (MHC) that travels throughout Sussex County. The MHC focuses on behavioral health and addiction medicine, with a strong emphasis on evidence‑based treatment standards. The NP will also provide preventive/primary health care and social healthcare services.

The MHC serves underserved populations, including homeless encampments, recovery homes, churches, schools, employer sites, and other community venues. The goal is to bring care to patients unable to travel to a Beebe facility.

Responsibilities Clinical Responsibilities
  • Comprehensive patient care, including assessment, treatment planning and implementation, counselling and education of patients and their families.
  • Collect and record data in a comprehensive, accurate, and systematic manner.
  • Analyse patient data and formulate an individualized plan of care based on assessment and needs.
  • Demonstrate compassion for individuals with mental health and substance use disorders and support patients during their recovery journey.
  • Work with patients to identify treatments that provide a safe and fulfilling life.
  • Provide primary care directly on the mobile health clinic, emphasizing accessibility, health promotion, illness prevention, and continuity of care for patients presenting with minor and complex health issues.
  • Complete physical examinations and perform or order appropriate diagnostic tests.
  • Manage common acute and chronic illnesses, including minor trauma, episodic problems, and chronic disorders.
  • Administer vaccines according to accepted guidelines and prescribe medications as appropriate.
  • Prescribe medication‑assisted treatment (MAT), including buprenorphine.
  • Maintain knowledge of clinical protocols and best‑practice guidelines.
Quality Of Service
  • Document all patient encounters completely and in a timely manner.
  • Adhere to clinical practice guidelines adopted by Beebe Healthcare.
  • Keep medical knowledge current by attending at least 20 hours of CME per year.
  • Maintain compliance with clinical and regulatory standards, including Joint Commission documentation requirements.
  • Demonstrate respectful interactions with patients and families.
  • Participate in performance improvement activities of Beebe Healthcare and the Mobile Health Clinic.
Teamwork
  • Collaborate with the multidisciplinary team on treatment, follow‑up, and plan evaluation.
  • Assist mobile health clinic staff willingly in difficult situations.
  • Show constructive problem‑solving with colleagues and mobile clinic staff.
  • Support established policies and procedures, and contribute to policy formulation.
  • Complete required documentation for referrals and consultations.
  • Return patient telephone calls within an appropriate time frame based on needs.
Miscellaneous
  • Demonstrate excellence in both internal and external customer service.
  • Understand and communicate HIPAA compliance, corporate compliance, and client confidentiality.
  • Ensure compliance with local, state, and federal regulations and accrediting standards.
  • Adhere to the National Patient Safety Goals defined by the Joint Commission and Beebe Healthcare.
  • Complete other duties as assigned.
Teaching & Community Involvement
  • Promote awareness, enrollment, and utilization of the Mobile Health Clinic among community partners and staff.
