General Description

A State Home Care RN assesses clients for services, determines clinical eligibility for programs, and develops care plans. The primary goal is to support individuals who receive services in the community so they can continue living in their own homes. Duties include data entry into a computer database, communication and consultation with family members, other Aging Services of North Central Massachusetts staff, and community professionals.

ROLE AND RESPONSIBILITIES
  • Participates as an integral member of a home care team along with case managers and other related personnel, assesses and/or authorizes services to be delivered by long‑term care agencies to promote coordination and cooperation in forming cost‑effective and quality service plans. The ASAP RN will participate in determining clients’ initial and continued eligibility for Home Care and Medicaid‑funded services.
  • Participates in the Personal Care (PC) Service Program according to Executive Office of Aging & Independence regulations. The SHC RN assesses clients for personal care, develops the PC care plans, and monitors the plan on a regular basis for appropriateness of the care plan and amount of service.
  • Conducts the Clinical Assessment and Eligibility screenings for Medicaid recipients or applicants:
    Acute and Chronic Hospital Nursing Facility Pre‑Admission, Community screenings, Adult Day Health screenings, and Initial and Reassessment Waiver screenings.
  • Participates in interdisciplinary RN/CM team meetings and case conferences.
  • Performs other duties as assigned.
  • Fosters an environment that facilitates professional collaboration among inter and interdisciplinary colleagues.
  • Builds and maintains a collegial environment that supports positive group processes.
  • Serves as a change agent within the organization.
  • Demonstrates leadership skills in decision‑making, problem‑solving, and teamwork and role‑models those characteristics.
  • Prepares self‑development plans, sets professional goals.
QUALIFICATIONS AND EDUCATION REQUIREMENTS
  • Graduate of an accredited school of nursing.
  • Minimum one year nursing experience required; community experience preferred.
  • Excellent computer, organizational, interpersonal, written, and verbal communication skills essential.
  • They should have experience and interest in the elderly population and/or community health. They should possess good communication, organization and time management skills; good nursing assessment skills; good judgement and decision‑making skills; and excellent documentation.
  • Knowledge of basic medical, mental health, social service delivery systems, as well as acceptable record‑keeping practices is essential.

A valid driver's license and a legally insured and registered motor vehicle are required; a copy of current license and registration will be on file in the personnel record.

Employment is contingent upon successful completion of pre‑employment physical, CORI (Criminal Offender Record Information), SORI (Sex Offender Registry Information), TB (tuberculosis) screening.
