We are hiring part‑time and per‑diem Registered Nurse (RN) Case Managers to join our dynamic care team at Hospice of Western & Central Massachusetts in Worcester County. This is an on‑the‑road position that requires the candidate to have insured and reliable transportation. Weekly pay. 8 a‑4:30 p. $48 – $50 an hour.

The RN Case Manager plans, organizes, and directs hospice care to our patients across the region and directs services to meet the needs of the patients and their families/caregivers.

• Completes an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs.
• Assess and evaluate patient status.
• Completes, maintains, and submits accurate and relevant clinical notes regarding patient's condition and care given.
• Communicate with the physician regarding patient needs and report changes in conditions.
• Supervise ancillary personnel.
• Develop care plans that establish goals, based on nursing diagnosis, and incorporate palliative nursing actions.
• Work in cooperation with the family/caregiver and hospice interdisciplinary team to meet the emotional needs of the patient and family/caregiver.
• Ensure equipment and other necessary services are available.

• Must hold an active and unencumbered RN license in the state.
• Hospice, home care, or acute care experience preferred.

• Must be willing to travel.
• Must be able to read, write, speak, and understand the English language.
• Must be a licensed driver with an insured automobile in working order.
