MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs, allowing seniors to receive quality care while staying active in their community and living in their own homes. Our programs are staffed with licensed nurses and experienced caregivers trained to provide individualized support.

We’re looking for dedicated Certified Home Health Aides (HHAs) to provide in‑home support and personal care to individuals who rely on our services to live safely and independently. This is a meaningful opportunity to build trusted relationships, promote dignity, and enhance quality of life while enjoying flexibility and professional growth.

• Provide assistance with daily living activities including bathing, dressing, grooming, eating, and mobility
• Build positive, therapeutic relationships with individuals and families to ensure quality care and support
• Support life skills, personal development, and independence goals outlined in care plans
• Help with light housekeeping, meal preparation, and errands as needed
• Maintain accurate progress notes and communicate effectively with your supervisor
• Attend team meetings and required trainings
• Report any safety or care concerns in accordance with company policy

• High school diploma or equivalent
• State‑issued Home Health Aide certification or CNA license required
• Valid driver’s license and reliable personal vehicle
• Compassionate, dependable, and professional demeanor
