Overview
Clinical Care Advisor - Prevention and Preservation – North Carolina residency is required.
Location:
we are currently seeking people throughout the State of North Carolina in the following DSS Regions. Travel within your assigned DSS region is required. When you are not in the field, you will work virtually from your home. These roles are statewide field-based and require you to interact with patients, members, or providers in person four to five days per week.
This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands-on engagement.

We are partnering with North Carolina DHHS to operationalize a statewide Medicaid Plan designed to support Medicaid-enrolled infants, children, youth, young adults, and families served by the child welfare system so they receive seamless, integrated, and coordinated health care. Within the Children and Families Specialty Plan (CFSP), members have access to physical health, behavioral health, pharmacy, I/DD services, long term services and supports, unmet health-related resource needs, and integrated care management.

- Coordinate operations and workflows related to case management activities in support of specialty programs (e.g., Foster Care).
- Serve as coach; perform case management telephonically and/or by home visits within the scope of licensure.
- Manage overall healthcare costs for the designated population via integrated physical and behavioral health case management and whole-person health.
- Manage the most complex cases and provide support to Special Programs case managers.
- Provide compassionate support to children and families involved in the CFSP Foster Care system with emphasis on strengthening family bonds and delivering services to those at risk of entering foster care.
- Conduct assessments to identify individual needs and develop comprehensive care plans to address objectives and goals.
- Support member access to appropriate quality and cost-effective care and modify plans as needed.
- Coordinate with internal and external resources to meet identified needs of the member in terms of integrated whole-person care and social determinants of health.
- Maintain knowledge of the system-of-care philosophy and a network of community-based services and supports.
- Build partnerships with designated populations and their families, addressing cultural and linguistic needs to help them function better at home and in the community.
- Evaluate health needs and identify applicable services and resources with members and families; provide education, medication reconciliation, and facilitate follow-up arrangements.
- Collaborate with state agencies as required.
- Identify and arrange resources to support follow-up care and ensure seamless service delivery.
State-specific requirements and licensure
For the State of North Carolina, in accordance with federal/state law and scope of practice:
- Requires a MSW/MA in social work, counseling, or a related behavioral health field, or a nursing degree with at least 3 years of clinical experience in social work/counseling with broad experience in complex psychiatric and/or substance use disorders; or an equivalent combination of education and experience.
- Requires an active, current, and valid license as an RN, LCSW (as applicable by state), LMHC, LMFT, LPC (as allowed by applicable state laws), or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states.
