- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Makes referrals to other hospice disciplines as needed or documents rationale for not doing so.
- Develop, prepare, and maintain individualized patient care progress records with accuracy, timeliness, and according to policies.
- Record pain/symptom management changes and outcomes as appropriate.
- Teach patients, families, and caregivers appropriate patient‑care techniques.
- Regularly communicate with clinical manager and care team.
- Perform on‑call responsibilities and provide services to patients/families as assigned.
- Other duties as assigned.
- One year of nursing experience.
- Current RN license specific to the state(s) you are assigned to work.
- Current CPR certification.
- Valid driver’s license, reliable transportation, and liability insurance.
