• $83k – $85k annually

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Makes referrals to other hospice disciplines as needed or documents rationale for not doing so.
• Develop, prepare, and maintain individualized patient care progress records with accuracy, timeliness, and according to policies.
• Record pain/symptom management changes and outcomes as appropriate.
• Teach patients, families, and caregivers appropriate patient‑care techniques.
• Regularly communicate with clinical manager and care team.
• Perform on‑call responsibilities and provide services to patients/families as assigned.
• Other duties as assigned.

• One year of nursing experience.
• Current RN license specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and liability insurance.
