ACHC is a Federally Qualified Health Center and licensed primary care clinic. We provide Family Medicine and Dental Care, with additional specialists in Behavioral Health, Chiropractic, Internal Medicine, Neurology, Pediatrics, Podiatry and Optometry. ACHC Clinics are located across Fresno, Kings and Tulare counties.
The Community Health Worker (CHW) is responsible for educating members of the community about health and wellness issues and practices. In this position, you may provide literature, organize events, or communicate directly with community members to teach them about disease prevention, how to access medical care, and how to develop healthy habits. In addition to providing health information to the population, you may work with ACHC healthcare team, community stakeholders, and other health outreach workers to develop a plan for outreach programs and campaigns.
ESSENTIAL DUTIES AND RESPONSIBILITIES- Responsible for establishing trusting relationships with community members within the rural communities while providing general support and encouragement.
- Follow up with community members with identified resources and/or education materials.
- Participate in community outreach events hosted by ACHC and/or invited to.
- Provide referrals for services to community agencies as appropriate.
- Effectively communicate ACHC’s mission to community members.
- Knowledgeable about community resources appropriate to the needs of patients/families.
- Act as a patient advocate and liaison between the patient/family and community service agencies.
- Attend regular staff meetings, training courses and other meetings, as requested.
- Maintain HIPAA compliance at all times.
- Drive to other sites and locations frequently.
- Maintain a strong level of confidentiality due to the sensitivity of materials and information handled.
- Act in a professional manner and consistently demonstrate and promote the values of respect, honesty, and dignity for the patient, families, and all members of the healthcare team.
- Maintain excellent working relations with patients, visitors and staff.
- Other duties as assigned.
EXPERIENCE
- Experience working in a multi-cultural setting.
- Experience working in a community-based setting for at least 1 to 2 years preferred.
- Previous Electronic Health Record (EHR) experience preferred.

- Ability to maintain regular, predictable, and punctual attendance.
- Proficient with Microsoft Office Suite and other computer-based platforms.
- Understand the community served and how this position connects to the community.
- Excellent verbal and written communication skills, including, but not limited to in-person, texting, telephone and email.
- Ability and willingness to provide emotional support, encouragement and motivation to patients.
- Ability to work independently and to carry out assignments to completion within parameters of instructions given.
- Ability to prioritize and perform functions with minimal supervision.
- Ability to work with diverse populations.
- Excellent organizational skills and attention to detail, resulting in a high-level of accuracy.
- Ability to listen to and comprehend communication from community partners, staff, clients, community members, and others.
EDUCATION REQUIREMENTS
- High school graduate or equivalent required; associate’s degree in business administration or related field preferred.
OTHER REQUIREMENTS
- Required:
Valid, properly classed driver license with a safe driving record. - Due to insurance and regulatory requirements, employee must be 21 years of age or older.
- Position requires night and weekend work, up to 20% of the full-time employee position.
- Written and oral fluency in both English and Spanish is preferred.
