We are now delighted to be able to offer 4 vacancies within the Young Adult Disabilities team.

We have some exciting news!

As part of our commitment to expanding support for young people, three brand‑new Social Worker posts have been created in our Young Adult Disabilities Team. In addition, we also have one existing vacancy for a Social Worker – making this a fantastic opportunity to join a growing team dedicated to making a real difference in the lives of young adults with disabilities.

The Young Adult Disabilities team (YAD) is a county‑wide service that supports young people with an Education Health and Care Plan (EHCP), who are likely to have needs for care and support as an adult to ensure those eligible social care needs will be met when they become 18.

The Care Act stipulates that preparation for adulthood should begin well before a young person reaches the age of 18. It is therefore crucial that the YAD Team can begin the process of co‑working and complete the adult social care assessment and support plan at an appropriate stage prior to the young person’s 18th birthday.

Good preparation for adult life needs to start early and high aspirations are crucial to success. The focus should be on the young person’s strengths and capabilities and the outcomes they want to achieve.

To plan successfully, all services involved in the young person’s life need to actively engage and support the planning for adulthood. An essential part of your role will be working collaboratively with children’s services, education, and health.

YAD team social workers focus on the completion of assessments in line with The Care Act 2014, safeguarding vulnerable adults, completing Mental Capacity Assessments/Best Interests decisions and the preparation of court reports.

Our team is friendly and supportive, with regular supervision and our employees have manageable workloads. The team hold weekly Group Supervision Meetings, to promote the best possible outcomes for the people we work with across Leicestershire. We provide a comprehensive induction package with ongoing access to bespoke learning and development opportunities. This includes tailored continuous professional development sessions and access to academic research and practice expertise to develop skills, experience and inform our employee’s practice.

About

You

To apply for this post, you must:

  • Be a qualified Social Worker and registered with Social Work England.
  • Have knowledge of social work methodologies, social care policies such as person‑centred approaches and asset based social work, and relevant legislation including The Care Act, Mental Health Act, and Mental Capacity Act.
  • Have experience of effective assessment and risk management delivery, and multi‑agency/multidisciplinary working and/or hospital discharge processes.
  • Have experience of partnership working with adults to identify and meet need with creative and pragmatic solutions.
  • Be a confident communicator who is skilled in diplomacy, persuasion, assertive and conflict management and able to build effective working relationships.
  • Be able to work proactively individually but also as a team member.
  • Have good prioritisation and time management skills and have a valid UK driving licence or access to a vehicle.
