The community connector is a brand‑new role within the Leicestershire Partnership Trust, Mental Health Planned Care Services to help support people to achieve better mental health and wellbeing, increasing independence and reducing reliance on services. This role is part of a wider transformation project in Leicester, Leicestershire, and Rutland to redesign community mental health care, with the aim of supporting more people closer to home, reducing the stigma around mental health and enabling existing services to work together more effectively.
Based in the City West Neighbourhood Mental Health Team they will link in with a wider integrated team of voluntary sector, local authority, and primary care staff.
The post‑holder will work with adults and older adults within the neighbourhood teams – a service supporting individuals with Serious Mental Illness. Under the direction of the MDT, the post‑holder will assist in providing a professional, efficient and seamless service, being the first point of contact for individuals referred into the neighbourhood team. The role involves delivering high‑quality individual sessions to patients, providing advice and information to promote healthier lifestyles and recovery, assessing risk and needs, developing appropriate intervention plans through brief interventions, and linking and supporting service users to voluntary and community sector organisations as needed.
As the role is new, the successful applicant needs to be innovative and proactive in engaging service users, forming links with local external agencies, and communicating effectively with individuals who may have complex needs. The successful candidate will also be expected to demonstrate the Trust’s values, live up to LPT’s Leadership Behaviours and undertake detailed holistic assessments of needs to identify therapeutic interventions.
