We have an excellent opportunity for Senior Care Coordinator to join the PIER team within the directorate of Adult Mental Health. The PIER (Psychosis Intervention Early Recovery) team serves Leicester, Leicestershire and Rutland for ages 14 - 65, supporting individuals experiencing their first episode of psychosis.

The team provides intensive support to service users and their families/careers for up to 3 years.

The Senior Care Coordinator will hold a caseload typically between 15 and 20 service users and work closely with other disciplines within the team (Medical, Psychological Therapies, Psychology, Support staff, admin and other care coordinators). The PIER team also links with other NHS services, statutory services and the voluntary sector.

Responsibilities
  • The successful candidate will work autonomously, taking responsibility for care, quality, and clinical activity for a designated caseload of service users.
  • Contribute to care quality, performance and clinical activity by providing professional leadership and guidance, clinical advice, and management in relation to the key dimensions of health care quality: patient safety, effectiveness of all health care interventions, patient experience, and the environment in which care is delivered. Participate in the effective clinical leadership of the team.
  • Actively contribute to the improvement of service and quality of care provision to meet the needs of service users and their families; act as a role model and participate in the education, development and mentorship of other staff.
  • Collaborate with other clinicians and within wider health, social care and voluntary sector communities as appropriate; promote the role of the Mental Health Practitioner to all stakeholders.
  • Foster a culture of networking in an open and transparent approach with internal and external partnerships and between all stakeholders.
  • Responsible for the assessment, planning, implementation and evaluation of programmes of evidence-based care for a group of service users according to changing healthcare needs and varying levels of complexity.
