What you will do:

Work collaboratively within the Integrated Locality Teams to uphold the health and wellbeing of Leicester’s residents, sharing responsibility with health colleagues.

Engage with service users and carers to assess needs, identify outcomes, and plan effective support.

Utilise your skills to carry out clear and complex assessments, compiling comprehensive plans and ensuring these are monitored and reviewed effectively.

Develop detailed reports for Tribunals and Manager’s panels, ensuring accurate documentation and case recording.

Apply your experience of working with adults with learning disabilities, mental health issues, older people, or those with physical, sensory, or substance misuse challenges.
