Principle Duties:
  • Admits new clients to private duty services and re-evaluates client needs.
  • Develops the plan of care.
  • Performs skilled nursing services as requested by the client and as prescribed by the healthcare practitioner.
  • Communicates with the physician regarding the client’s needs and reports any changes in the client’s condition; obtains/receives physician’s orders as required.
  • Communicates with community staff to strategize about how to improve client outcomes.
  • Performs supervisory visits to evaluate client’s satisfaction with care and services provided.
Qualifications:
  • Graduate of an accredited school of nursing.
  • One (1) to two (2) years of recent acute care or home care experience preferred but not required.
  • Current RN/LPN licensure in the state.
  • Current CPR certification.
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
  • Has the ability to read, follow written instructions, and document services provided.
  • Is self-directed with the ability to work with little direct supervision.
  • Is kind, reliable, and compassionate.
