We are seeking an experienced Registered Nurse (RN) to serve as a Clinical Manager within our Home Health division. This leadership role oversees patient care from admission through discharge while supporting field clinicians and ensuring regulatory compliance.

The Clinical Manager plays a central role in coordinating patient care, supervising staff, and maintaining quality and compliance standards across the agency.

• Oversee and coordinate patient care from admission through discharge
• Conduct and/or review patient assessments and care plans
• Ensure physician orders are obtained, reviewed, and implemented timely
• Supervise and support field clinicians (RNs, LPNs, aides, therapists as applicable)
• Review clinical documentation for accuracy, completeness, and compliance
• Monitor quality assurance, outcomes, and clinical performance indicators
  
  Collaborate with intake, billing, and administrative teams
• Assist with audits, surveys, and regulatory readiness
• Address clinical issues, incidents, and patient/family concerns as needed

• Active Registered Nurse (RN) license in the state of Florida
• Minimum 2–3 years of home health experience preferred
• Strong knowledge of home health regulations and clinical standards
• Experience with care coordination and clinical documentation
• Excellent communication, organization, and problem-solving skills
• Comfortable using EMR/home health software systems

• Prior experience as a Clinical Care Manager, Case Manager, or Field Supervisor
• Familiarity with Medicare and Medicaid requirements
• Experience preparing for surveys or audits
